Presented by Michelle Wilson

Michelle is a Registered Nurse with a diverse clinical experience working in Australia and the UK.  Her current role at Barwon Health, Victoria is coordinating the Remote Patient Monitoring and HealthLinks teams.  The majority of her career has been in cardiology and chronic disease management.  Roles in these areas have included research, clinical case management and project development.  Michelle has also worked for Deakin University for the past decade, in both the School of Nursing and Psychology.

The presentation will reflect the journey and learnings of the Remote Patient Monitoring program at Barwon Health.  Commencing as a Proof of Concept project in 2013, the team has grown and evolved.  Some of the challenges, solutions and outcomes will be discussed, as well as possibilities and future directions.
