Nutrition plays an important role for patients with chronic diseases. Modifiable risk factors such as diet can help with prevention as well as management of chronic disease. Increasingly, digital health technologies are being used for planning, tracking, and evaluating people’s nutrition and diet. Mobile apps can provide information on healthy choices or suggest recipes, and they can also be used be used to track progress of patients.
The COH is working to co-design, implement and evaluate digital health models of care and evidence-based programs for nutrition care.
