Goondir Health is an Aboriginal Community Controlled Health Service providing primary health care and related health services to clients in the Darling Down's townships of Dalby, Oakey and St George and surrounding areas. The Virtual Health Service (VHS) offers remote physiological monitoring and management for clients with chronic disease. Goondir have 198 clients enrolled in the VHS.
This partnerships will evaluate the clinical effectiveness, cost effectiveness, organisational impact, and client preference of this new model of care delivery. The evaluation will be undertaken over short, medium, and long term. Initial resulst show the clients found the VHS system easy to learn and use, and the VHS has been very successful at improving client empowerment, engagement and increasing interactions with their health care professional.
